Children’s Community SupportFamily Support Wales offers community support to children of all ages.  We understand that children with complex needs and diagnosis may require specific support tailored to their needs.  We also understand the importance of the family involved every day supporting children who live day-to-day with complex needs or display challenging behaviour.

Our support starts with the very basics and that is getting to know you and those requiring support. This will help us have an understanding of your needs and what will help you, we can arrange to visit you at a time to suit and complete some assessments that will feed our support process. Once we understand your circumstances and requirements, we look at how our team can support you. We have a core team of professionally qualified individuals who will oversee your support needs and service requirements.

We will work in partnership with you and other professionals to deliver the best support service possible to ensure we meet the requirements of all your family.

As a team, our roles are to assess, implement and oversee service delivery. We are able to offer services to support challenging behaviour and complex needs for children.

Our support staff are trained and supported to deliver a support service that will encourage improvement with life skills, social skills and community inclusion. To ensure we achieve high standards of support, each service is assessed with the intention of building a pod of dedicated support staff to each service. This pod of support staff will bring long-term continuity to the service that will encourage those we support to achieve their specific outcomes.

Our staff are trained in PBM (Positive Behavioural Management) and PBS (Positive Behavioural Support) to ensure the safety of those we support. Our staff will work to recognised support plan in relation to how we support your child or family member through periods of behavioural difficulties while involving them in community activities.

We offer various other services including:

  • Community involvement
  • Social activities
  • Positive mentors
  • Life skills
  • Confidence building
  • Social inclusion
  • Companionship
  • Support to enjoy time in your community

Family Support Wales support workers are trained to use our holistic model to enable the children we support to access the community and enjoy all that’s on offer to them. This means taking a person-centred approach that focuses on understanding the person being supported; how they like to be communicated with, what makes them tick, what frustrates them and what they want to achieve. This takes time and patience, but when a support worker has the answers to these questions, the impact they can have is immeasurable.

Our Holistic Model of Support

The Holistic Model is based on the concept of Person-Centred Active Support (PCAS). This is a way of supporting people so that they are engaged in meaningful activity and relationships as active participants. And as a result, they exercise more control over their lives and experience greater levels of inclusion, independence and choice.

Providing person-centred support requires us to understand the people we support. This may seem obvious, but it is important to note that this understanding is not limited to knowing how to meet people’s assessed needs. Person-centred support requires an understanding of what is important to each individual, what their positive qualities are and what they want for their future.
