When you request our services from our first meeting you will receive comprehensive support and guidance.
One of our senior staff will visit you and introduce you to Family Support Wales. You will receive an information pack that will have details of the company and what we do. During our first meeting we will complete a general assessment of need that will include getting to know more about the person who will receive the service, their specific needs, personality and what you as a family would like from the service delivery.
We will complete necessary formalities to ensure we have your consent to store and share information, use our transport when travelling with your loved one and other important and vital forms to ensure the safety of you and your family.
You will have a dedicated community support manager who will visit you and the person receiving support. They will spend time with your family to get to know them and what is important to them. During this period, we will be looking at matching up the best team possible to support your service requirements. We look at matching personalities, interests and importantly skills and knowledge to ensure we deliver a high standard of support and care.
With your help we will build a team dedicated to your service needs. You will always have direct access to your community support manager and senior team via their works mobile phone. You will also be able to sign up for a phone app that will upload the times of your agreed support sessions and the names of those who will be attending.
Our support and care workers will feedback via our website details of our time on service. This will allow you to keep up to date with what we do when supporting those you love. We also have a monthly ‘outcomes’ monitoring form that will help us keep focussed on those important aims and objectives that your family are keen to see come to fruition for those we support.
We will support you and your family in every way possible, that includes attending community working groups, strategy meetings and local Authority service reviews.
Our aim is to ensure you have a strong team that is trained technically to meet the needs of the service, to emotionally meet the needs and to positively impact on your whole family. We will build a strong and close working relationship with you and your family that will be there through the many ‘ups & downs’ that life can bring.
You won’t see an unknown support worker at your door, when we call you will see a friendly face that you know well, and trust.
